Progress Notes: Done is better than perfect

For a lot of mental health therapists, these promises don't actually help. They backfire.

Why aren't your progress notes getting done?

I've worked with hundreds of therapists who genuinely want to stay on top of their progress notes. They know it matters. And they still can't figure out why the case notes aren't getting written.

It's not a lack of effort or intelligence. Content might be part of it, but that's rarely the whole story. Time management might be part of it too, but that's not the whole story, either.

Why perfectionism makes progress notes so hard to finish

We write progress notes in isolation for good reason - confidentiality matters. But that same isolation can leave you stuck, staring at a screen, second-guessing everything you type.

Did you include enough interventions? The right ones?

Did you write too much about your client's response, or not enough?

Is there enough detail to support medical necessity, without writing out your client's entire life story for insurance to read?

It's exhausting! And that exhaustion and overwhelm is exactly what leads to avoiding progress notes altogether... which leads to falling further behind... which leads to additional stress about progress notes. 

Why progress notes trainings don't always help

It's hard to absorb new information when your nervous system is already on high alert. What actually helps first is the calm that comes from knowing you're not the only one struggling with this.

A grounded, curated community can take all that collective anxiety around progress notes and turn it into something calmer. Yes, I really just used the words "ease" and "progress notes" in the same sentence.

Once you're in that kind of contained, supportive space, you can actually take in the guidance that applies to you and let go of what doesn't. From there, you can accept something simple but powerful: a completed progress note is better than a perfect one, since a perfect note doesn't really exist anyway.

That's how you build real confidence. And confidence is the antidote to perfectionism.
